By attending this event, I hereby authorize and grant Credit Union West (“CUW”) and Desert Diamond Arena ("DDA") permission to use, copy, reproduce, publish, distribute, and/or modify photographs and/or video of me and any attendees listed in this submission, including minor children, in all forms of media and marketing materials. I understand and agree that such photographs and/or video may be used by CUW and DDA for any purpose, including but not limited to brochures, newsletters, advertisements, flyers, displays, websites, Internet, and social media, without further notice to me. I acknowledge that images or videos may be edited, altered, copied, exhibited, published, or distributed, and I waive the right to review or approve the final product in which my likeness or that of my minor child appears. I further understand and agree that neither I nor any listed attendee, including my minor child(ren), will receive compensation for such use. CUW and DDA's may use identifying information limited to name, age, and city of residence. I acknowledge that once images or videos are posted on CUW and DDA's website, Internet, or social media, they may be downloaded, shared, or reused by others outside CUW and DDA's control. I hereby waive all rights and release CUW and DDA, its successors, employees, agents, directors, and officers from any and all liability, claims, or causes of action, whether known or unknown, including but not limited to claims of libel, defamation, or invasion of privacy, arising out of or related to the use of such photographs and/or video of me and/or attendees listed in this submission, including minor children.
